OC Connections offers individualised support which can be provided in a person’s family home, or in other accommodation within the community. This support varies from person to person and is based on identified needs and aspirations and what services are necessary to best meet these. Crucial to an individual’s growth is the ability to have choice and control over what support is needed and how it will be provided.
We offer programs to develop personal skills, maximise independence and assistance to access the local community.
Our Accommodation Services use a self-directed approach that provides people with the choice and control to determine what support they need to live the life they want. This approach complements the NDIS ideals of inclusion and community participation. People can use this approach to manage their accommodation, community connections and access to activities.
A self-directed approach offers individuals the opportunity to:
- Determine for themselves what supports they want
- Determine how, and with what service provider, their funding money will be spent
- Have flexible support that is tailored to their needs.
Self-directed support is the key means by which to achieve life goals or pursue interests. These supports are usually identified within a person’s own particular plan and can include a combination of informal supports, assistance provided through generic or community services accessed by the whole community or via funded disability support.
