The OC Connections’ NDIS Intake and Transition team focus on the onboarding of new participants to our services, as well as the transition of current participants from one NDIS plan to the next.
When you contact OC Connections to make an enquiry about our services, you will be in touch with our Intake and Transition team, who will help you navigate our available supports and determine if they will be the best fit for your goals, location, hours of support required, age, funding, transport requirements, or any other requirements that will impact your supports.
If you are wanting Support Coordination services, they will connect you with one of our Support Coordinators who can then assist you in finding the right services for you, either with OC Connections or with other disability service providers.
Once you have determined that OC Connections will be a good fit, the relevant OC Connections’ service area/areas will be in touch to arrange a site visit (for select services), meetings with our support teams, any trial periods (for select services), time to review your specific support documents and provide relevant quotes for your planners.
When the decision is made to come on board to OC Connections, the Intake team will work with you, your Support Coordinator If you have one, and our service teams to develop service agreements and ensure all appropriate services are in place and you the information regarding the frequency and cost of these services. Mari, Lily or Laura will then work with your support coordinator and support teams to ensure all actions required by OC Connections are in place to help you with upcoming plan reviews, new plan periods, extensions or funding allocations.
