At OC Connections, our Day Support program offers participants a range of activities, workshops and classes which are based either at our Oakleigh site, or out and about within the community.

Our programs aim to increase participants' independence, confidence and life skills. When it comes to choosing activities, participants and families can select preferred activities from our Activity Guide or alternate preferences can be discussed with us. Where required, support and guidance will be given in developing a plan that targets an individual’s interests and goals and fits with their preferred way of learning or socialising with others: some people like structure and routine, while others enjoy variety and new experiences – we’re happy to accommodate all choices.

We assist people to participate in a wide range of activities based on their individual goals and abilities. These goals are linked with the Disability Standards; Empowerment, Access and Engagement, Well being and Participation.

Key Workers

Every participate who joins our Day Support program is assigned a Key Worker who is responsible for supporting them to get the best out of their activities and time.

Our Key Workers will work with individuals and their families to create a plan that helps to identify interests and goals and will suggest activities to support these. Participants are encouraged to progress towards these goals at their own pace; more important is that every person lives the sort of life they want, with as much control and independence as possible.

Key Workers are a first point of contact for families to ensure that as needs change, the best supports are in place to manage and respond to these. We always encourage input from families, friends and other people who are important to the individual. 

Transition from School

The move from school to OC Connections (or other adult day support programs) can be daunting for young people. Some will transition quite easily, while others proceed more slowly. We understand this and provide a consistent team of staff members that offer participants continuity and stability. Our fully trained and skilled staff assist to make this transition as easy as possible. Once a person is comfortable with their new environment, they can then choose activities that focus on their interests. This approach is based on personal needs and in collaboration with the individual and their family/carer.

OC Connections also offers participants the opportunity to complete their Certificate 1 in Transition Education. The course focuses on independent living, vocational and employability skills with learnings offered in personal and social development and financial independence.

Administrative Fees

The administrative fee is $5.00 per day. This means that it will be $25.00 per week for someone who attends on a full time basis. This fee contributes to those administrative costs that are not covered by our funding from the Department of Health and Human Services (DHHS). For long absences the charges will be negotiated.

Activity Charges

OC Connections aims to provide activities to match every budget. Some of our activities are free, while others incur a charge based on the activity type and where it is held.

Additional to the activity charge, is a daily $5.00 administrative fee based on the number of days participants attend the centre (eg: someone attending five days a week will pay a $25 administrative fee). This fee contributes to administrative costs not covered by our funding from the Department of Health and Human Services (DHHS). For long absences the charges will be negotiated.
