Life Skills & Leadership

Through community interaction, volunteering, travel opportunities, and exploring the vibrant city of Melbourne, you will develop teamwork, organisational skills, leadership abilities, literacy, and independence. Fun, Fitness & Wellbeing Programs

Prioritise your health, fitness, and overall wellbeing while having fun with friends. Our engaging programs offer a wide range of activities to cater to different interests and abilities.

Join walking groups, go biking, engage in gym workouts, play golf, participate in ball sports, take a refreshing swim, or find your inner balance through yoga. 

 

Accessible Holiday House

Escape to Tootgarook Holiday House, a hidden gem nestled between Rosebud and Rye on the stunning Mornington Peninsula.

This fully accessible home is available year-round for private or group bookings.

The house is suitable for people with limited mobility, featuring accessible bathrooms, a shower-bed and portable hoists if required.

Extended hours and weekend support

At OC Connections, we go above and beyond to ensure that our support services are available when you need them the most. That’s why we offer support Monday to Saturdays, including evening hours, providing you with the flexibility to access the assistance you require.

Whether you prefer individualised 1:1 support or the dynamic environment of group settings, we are committed to delivering comprehensive support that caters to your unique preferences and schedule.

We believe in being there for you every step of the way, ensuring that you have the support you need, whenever you need it.

Centre-based and out in the community

Activities are offered on site at our Oakleigh or Cranbourne West locations, as well as in local Community Hubs, and out in the community.

You will often see OC Connections participants and support workers exploring Melbourne. We have regular trips to the zoo, markets, cafes, parks, libraries, sporting facilities, art galleries, farms, schools and shopping centres.

Keyworker model of support

Every participant who joins our Day Support program is assigned a Key Worker who is responsible for supporting them to achieve their goals.

Our Key Workers are trained to work collaboratively with participants and their families to identify interests, set goals, create plans and provide the support needed to achieve them.

Key Workers are a first point of contact for families to ensure consistent communication so that as needs change, the best supports are put in place to manage and respond to them. 

Transportation

To ensure you can participate fully, we provide support for public transport travel and can assist in arranging taxis or private bus transport.

Please note that transport arrangements may incur an additional fee, allowing us to provide you with safe and reliable transportation options.

Eligibility & Fees

Participants must be aged 15 years or over and receive funding through the NDIS.

For people who do not currently have funding in their plan there is the opportunity to self fund. 

OC Connections aims to provide activities to match every budget. Some of our activities are free, while others incur a charge based on the activity type and where it is held. Get in touch to find out more and discuss your options!
