Flexible In Home disability Support Services Tailored To Your Needs
At OC Connections, we understand that support needs vary for people with disabilities and to achieve the best outcome for you is to provide you with choice and control over your In Home Support.
OC Connections offers individualised in home care & support which can be provided in your family home or in other disability accommodation within the community and is tailored to your needs and goals. This ensures you receive disability support best suited to you so you can live the life you want.
How OC Connections Can Help
OC Connections’ In Home disability support services work with you so we can find the right supports to allow you to live where and how you choose.
We offer you and your support network the opportunity to:
- Determine what supports you want
- Determine how your funding will be spent
- Have flexible support that suits your needs and goals
- Find the right support personnel to help you reach your goals
Our In Home supports can include a mix of informal supports, help through community services, or from funded disability supports (NDIS). We also offer support to develop your personal and independence skills. These include, but are not limited to:
- Meal preparation
- Grocery shopping
- Support to attend appointments
NDIS Funding for In Home DISABILITY Supports
In Home Supports can be funded by NDIS as part of your Core Supports. It is best that you talk with your NDIS Planner at your meeting to discuss the disability support services you require within your home to meet your needs and goals and to assist you live the life you choose.
Once you have NDIS funding within your NDIS plan, you can talk to our Community Living staff about how OC Connections can work with you to meet your support needs.

OC CONNECTIONS MANAGED ACCOMMODATION WITH IN HOME SUPPORT
OC Connections offers individualised support which can be provided in your family home or in other accommodation within the community. Additionally, OC Connections also manages disability accommodation within South East Melbourne which includes in home support.
iN HOME SUPPORT VACANCY IN MOUNT WAVERLEY
A vacancy exists in our Mount Waverley managed accommodation for an NDIS funded individual with in home support funding.
This property is a shared living environment, with three other residents aged 46 – 55. It is suitable for housemates who wants to socialise and spend time together.
Supports are minimal across seven days and are shared and claimed equally.

What is In Home support?
In Home support is support provided by a paid disability worker in your home. It allows you to get the home help & support you need in your own home to help you live the life you want to live

Does the NDIS cover In Home support services?
It does, under Core Supports. Your supports should be discussed at your planning meeting. Your plan will feature these supports as part of your agreed goals.

What is the difference between In Home support and Supported Independent Living?
This is about the level of funding and whether you are living alone or with others. Both Supported Independent Living (SIL) and In Home support can be provided in your home, however In Home support is an individualised support, whereas SIL can be shared.
All SIL support at OC Connections is currently provided in SDA environments. In Home support at OC Connections is more flexible and we can provide this in a variety of environments, including care homes and private homes.

Who is eligible for In Home disability support services?
Eligibility for In home support is determined by the NDIS based on reasonable and necessary requirements for each individual. We recommend you discuss this with your planner to determine the right supports for your needs.
