About the OC Connections Day Support program

We offer participants a range of activities, workshops and classes to increase independence, confidence and life skills. Our programs are held at a number of different venues including OC Connections, 773 Warrigal Road Oakleigh and community based facilities.

Key Workers

Every participant who joins our Day Support program is assigned a Key Worker who is responsible for supporting them to achieve their goals.

Our Key Workers work with individuals and their families to create a plan that helps to identify interests and goals..

Key Workers are a first point of contact for families to ensure that as needs change, the best supports are in place to manage and respond to these.

We always encourage input from families, friends and other people who are important to the individual.


Participants travel to and from OC Connections by private transport, taxis or one of our buses. Transport to and from community activities is provided by OC Connections from 773 Warrigal Road Oakleigh.


Participants must be aged 18 years or over and receive funding through the NDIS. For people who do not currently have funding in their plan there is the opportunity to self fund.


OC Connections aims to provide activities to match every budget. Some of our activities are free, while others incur a charge based on the activity type and where it is held.
